The Continuum of Care (CoC) is a program designed to promote community wide commitment to the goal of ending homelessness.
The Montgomery County Office of Housing and Community Development is the Collaborative Applicant responsible for facilitating Your Way Home Montgomery County’s PA-504 Continuum of Care (CoC) Program Competition issued by the federal department of Housing and Urban Development (HUD), including the review, rating and ranking of new and renewal projects that provide housing and services for residents experiencing or at imminent risk of homelessness using these federal dollars.
PA-504 CoC HUD FY2025 Grant Competition
The U.S. Department of Housing and Urban Development (HUD) has published the Notice of Funding Opportunity for the FY 2025 Continuum of Care on November 13, 2025. The Your Way Home Montgomery County PA-504 Continuum of Care (CoC) is soliciting agencies to submit New, Transition, and Renewal project applications. Estimated funding amounts for each CoC have been released by HUD and can be found at: https://www.hud.gov/sites/dfiles/CPD/documents/CoC/FY25-CoC-Estimated-Annual-Renewal-Demand-Amount-Report.pdf
Eligible Renewal project applicants may submit applications for eligible renewal components in:
· Permanent Supportive Housing (PSH)
· Homeless Management Information System (HMIS)
New and Transition project applicants may submit applications for:
· Transitional Housing (TH)
· Supportive Services Only (SSO)
HUD will also earmark a fixed amount of potential funding allocated to the CoC for DV Bonus projects.
All project applicants must prepare to submit their individual project applications to HUD via e-snaps by end of day Friday, December 12, 2025. Please plan accordingly, as the deadline is firm and the application forms are not yet available in e-snaps.

Questions regarding information in the NOFO, Detailed Instructions, and Navigation Guides may be submitted to CoCNOFO@hud.gov; however, HUD cannot assist you with answering specific application questions.
Questions related to e-snaps functionality (e.g., password lockout, access to user’s application account, updating Applicant Profile, other technical issues with submitting the application) must be submitted to e-snaps@hud.gov.
The Your Way Home Montgomery County CoC supports prospective applicants and projects that support HUD’s new national priorities around treatment, recovery, required supportive services, advancing public safety, promoting self-sufficiency, and minimizing trauma. The Your Way Home Montgomery County CoC encourages applications from applicants that have never previously received CoC funds as well as from applicants that are currently receiving or have in the past received CoC funds, including faith-based organizations.
The HHS Office of Housing and Community Development in collaboration with the PA-504 CoC Governing Board reserves the right to request that applicant organizations submit adjusted project budgets based on the amount of funding made available by HUD and recommendations from the CoC Rating and Ranking Committee.
