About Your Way Home 

The Your Way Home Montgomery County Partnership to End Homelessness is the county’s unified and coordinated housing crisis response system for families and individuals experiencing homelessness or at imminent risk of homelessness. Programs

You Way Home Programs are run by contacted providers and include Street Outreach, Emergency Shelter, Rapid Rehousing, and Permanent Supportive Housing.

Your Way Home is a public-private partnership between Montgomery County Department of Health and Human Services, the PA-504 Continuum of Care, and contracted homeless and homeless prevention service providers.
