About Your Way Home
The Your Way Home Montgomery County Partnership to End Homelessness is the county’s unified and coordinated housing crisis response system for families and individuals experiencing homelessness or at imminent risk of homelessness. Programs
You Way Home Programs are run by contacted providers and include Street Outreach, Emergency Shelter, Rapid Rehousing, and Permanent Supportive Housing.
Partners
Your Way Home is a public-private partnership between Montgomery County Department of Health and Human Services, the PA-504 Continuum of Care, and contracted homeless and homeless prevention service providers.
Prevention
Homelessness prevention is an expansion of the work already done by Your Way Home, and targets those who are at risk of losing their housing.

Equity
Your Way Home is accessible to all eligible persons regardless of race, color, national origin, religion, sex, age, familial status, disability, actual or perceived sexual orientation, gender identity, or marital status.
First Time Homebuyers
Montgomery County offers a First Time Homebuyers Program that can provide up to $10,000.00 for the down payment and closing costs associated in purchasing a home within Montgomery County, PA!
Continuum of Care
The Continuum of Care (CoC) is a program designed to promote community wide commitment to the goal of ending homelessness.

Important Update! Supplemental Nutrition Assistance Program (SNAP)
according to the Pennsylvania state government, Starting October 16, 2025 SNAP benefits will not be paid until the federal government shutdown ends and funds are released to Pennsylvania. the state will notify SNAP recipients when payments can resume, and they will let you know when to expect SNAP payments.
