Your Way Home has significantly reversed the problem of homelessness in Montgomery County.  Homelessness was trending upward in the County since 2011, peaking at 464 individuals who were homeless on a single night in January 2013.  Since then, Your Way Home has reduced homelessness by 33%, to 310 individuals on a single night in January 2017.  

Research evidence--‐based and promising practices to prevent eviction and support housing stability for at--‐risk individuals and families.

  • Inform local policy and practice including allocation of financial and human resources; and to identify potential sources of funding to support these eviction prevention strategies
  • Inform the national dialogue on how to prevent homelessness

Your Way Home Montgomery County released a new research report on emergency shelter policies and procedures to guide our partners working to end and prevent homelessness in Montgomery County, PA. Through the generous investment of the HealthSpark Foundation, Your Way Home contracted with Diana T. Myers and Associates, Inc. to research emergency shelter policies and procedures within a coordinated entry crisis response system.

The Your Way Home Operations Manual is designed to provide information and guidance to Your Way Home service partners for the implementation of Your Way Home program activities.

This document is aligned with and organized to follow the major processes of Montgomery County’s unified housing crisis response system:

  • Coordinated Entry, including Call Center and Street Outreach activities

  • Housing Counseling

  • Emergency Shelter

  • Rapid Re-Housing

  • Transitional Housing

  • Permanent Supportive Housing
